At Wellbound Home Care our HHA’s are placed in the comfort of a child’s home to provide relief for the family from the day to day constant attention and care requird for children diagnosed on the spectrum or with an ID/DD. Our Home Health Aides are trained and certified in special needs pediatric care. They provide hands on personal care and skilled practices that go well beyond the limits of standardized home care.

At Wellbound we provide each one of our individuals with a dedicated case manager and Patient Advocate. Together their roles are to maintain the needs of the child or adult, as well to voice the concerns or questions of the parent or self advocate.

Our team of Case Managers and Registered Nurses are dedicated to coordinating the details and clinical implementation of care plans—-ensuring every patient receives the integrated care they need and all service expectations are fulfilled. To encourage optimal patient wellbeing, our Case Managers maintain open communication with the individual’s parent, doctor, and medical specialist, updating progress and patient evaluations throughout the care plan.

Our dedicated Patient Advocates work directly with our individuals, parents or guardians to provide assurance, guidance, support, and assistance while advocating for all necessary services.
