They will work with our Intake Team and collaborate with your doctors and healthcare providers to obtain necessary orders for your home care services, ensuring a smooth and stress-free experience.
3. Connecting You with the Right Care
Our intake scheduling specialists will schedule the assessments needed for Start of Care. They will also handle logistics and address any arising details, ensuring a smooth and stress-free start to your care.
4. Tailoring Your Care to Your Needs
Next, you'll meet your case manager, who will serve as your clinical guide throughout your child's care journey. Your case manager will work with your child's therapist, doctor, and CCO care manager to create a care plan tailored to your specific needs, ensuring you receive the right level of support.
